Horizon House will provide services for Out-of-School Youth (OSY) who have been labeled with serious emotional disturbance (SED), or have one or more mental health diagnoses and /or learning disabilities. They will target OSY with a GED or high school diploma but are basic skills deficient and want to advance to a post-secondary degree program. To this aim, the program will offer college bound classes on topics including study skills, time management, life skills, and research skills; assistance in completing college entrance applications and financial aid counseling; individualized academic tutoring and SAT preparation, career counseling; a mentoring program; and supportive services including linkages to mental and physical health services and drug and alcohol services both within Horizon House and other agencies; housing and financial assistance, benefits counseling, and disability management.
