Online Intern Request Form
The purpose of the Intern Request Form is to assist companies with clearly defining the intern's duties and responsibilities. Business Partnership Specialists will also use the Intern Request Form to match qualified candidates to a company's specific needs.
When completing the Intern Request Form
Keep in mind a few things...
- Identify the department and supervisor who will be responsible for the youth.
This is important because the youth will be instructed to contact this person if they are unable to attend work, have questions about lunch breaks, e-mail, etc. - Include a secondary contact
Sometimes the initial contact is not the day-to-day person responsible for the intern - Specify the tasks to be performed
- Be specific about work hours, dress code and other departmental policies that may arise
- Determine the hourly wage (refer to the chart on the WorkReady Information sheet on Wages and Program Costs)
The Job Description
A detailed job description may accompany the Online Intern Request Form. You will have the opportunity to submit one at the end of the Intern Request Form. Job descriptions should be given to the interns so that they are clear about their duties and responsibilities.
